Applications can be found below. Fill it out and bring it with you to open interviews on Tuesdays. (If you do not have access to a printer, please go to STEP 2.)
We encourage everyone to apply that is struggling with substance abuse addiction!
Open interviews every Tuesday at our administrative office 1215 Lake Drive, Cocoa, FL 32922 2p.m. – 4 p.m. There is no charge and no appointment is needed.
If accepted into the program, clients are required to get a physical, TB test and syphilis test prior to admission.
If not accepted into the program, referral sources will be given.
*Brevard County Jail: Applications for our program can be requested by the inmate from the Jail Liaison. Applications will be forwarded to us for review and phone interviews will be scheduled between the inmate and our Clinical Supervisor to screen for appropriate level of care.
*Other Institutions and Correctional Facilities: each facility has their own process for applications and interviews, please call for details.
